Completed forms can be returned to reception or emailed to (practice email – nelondonicb.nsmc@nhs.net). Once completed and returned, your information will be reviewed by a nurse and you will be contacted. If required a travel appointment will also be booked.
We are pleased to be able to offer a comprehensive foreign travel advisory service which includes the supply and administration of most of the vaccines, medications and health related products you will need for your travel plans.
Some patients, especially back packers and those travelling to Far East countries, may be asked to attend our travel clinic once the travel clinic forms are completed. Our travel trained practice nurses runs this clinic with support from Dr Burack.
There will be a charge for the following vaccinations if required:
- Rabies
- Japanese Encephalitis
- Cholera
- Hepatitis B
- Yellow Fever.
There will also be a charge for private prescriptions for travel drugs. We will advise you if any of these are required and inform you of the current prices for each vaccine. Payment must be made in full at the time of your appointment.
We accept cash, card or online payments. Please see the poster in waiting room for up to date prices and our website. All attendees of our travel clinic will receive an up to date health-related travel guide. We also offer a travel service to private patients. Details of fees are available from reception.

It is important to make this initial appointment as early as possible. Some vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
